We may collect information from you when you register on 5 Elements Therapy website (the “Website”), place an order with us using a 3rd party software, subscribe to a newsletter or email from 5 Elements Therapy, or respond to a survey or fill out a form.

Alternatively, you may visit and provide the requisite information through that channel. Note that you will be provided with a Wellness Living login username and password.
